Cardiology:
Dr. Afroz Hai is a Cardiac Electrophysiologist who is seeing patients at the Duly Health and Care location at 2359 Hassell Rd, Hoffman Estates, IL. Dr. Hai earned his medical degree at the University of Illinois College of Medicine, completed residency at Loyola University and completed a fellowship in Cardiology at the Hospital of Saint Raphael, Yale University and Medical Center.
Endocrinology:
Dr. Monica Patel is board-certified in Endocrinology and sees adult patients at 5207 Main Street in Downers Grove. Dr. Patel received her degree from West Virginia University, her residency in Internal Medicine at Rush University Medical Center and her fellowship in Endocrinology from Rush University Medical Center. Dr. Khan earned his degree at St. James School of Medicine, completed residency in Family Medicine at the University of Ottawa and completed a fellowship in Geriatrics at University of Toronto. Dr. Faizan Ali Khan is a Gastroenterologist currently seeing adult patients in Joliet and Lockport. Dr. Khan received his medical degree from the Chicago College of Osteopathic Medicine. He specializes in gastrointestinal cancer and inflammatory bowel disease. He maintains certifications and licensure in IL, MN, and IN. He is a member of the American Osteopathic Association Board of Gastroenterology and Internal Medicine.
Hospitalists:
Dr. Sana Chaudhry is a hospitalist who is board-certified in Internal Medicine. She received her degree from Ross University School of Medicine and completed her residency in Internal Medicine at the University of Florida – Jacksonville. Dr. Chaudhry collaborates and works closely with each patient’s primary care provider for a seamless transition of care at Advocate Good Samaritan Hospital, Edward Hospital, Elmhurst Memorial Hospital and Northwestern Medicine Central DuPage Hospital.
Dr. Timothy Long is a hospitalist who is board-certified in Internal Medicine. He received his degree from Michigan State University in the College of Osteopathic Medicine and completed his residency at Loyola University Medical Center. Dr. Long collaborates and works closely with each patient’s primary care provider for a seamless transition of care for adult patients at Elmhurst hospital.
Immediate Care:
Dr. Gretchen Sanford is board-certified in Emergency Medicine. She received her degree from the Midwestern University Chicago College of Osteopathic Medicine and completed her residency in Emergency Medicine at Resurrection Medical Center. Dr. Sanford currently sees patients of all ages at 430 Pennsylvania Ave. Suite 150, in Glen Ellyn.
Dr. Erika Havelka is board-certified in Emergency Medicine. She received her degree from University of Illinois and completed her residency at Advocate Christ Medical Center. Dr. Havelka sees patients of all ages at Duly Health and Care Immediate Care Centers in Bloomingdale, Glen Ellyn, Hinsdale, Lisle, Lockport, Naperville, and Westmont.
Dr. Steve Hall is board-certified in Emergency Medicine. He received his degree from the University of Illinois Medical School and completed his residency at Resurrection Hospital in Emergency Medicine. Dr. Hall sees patients of all ages at 808 Rickert Drive, Suite 103, in Naperville.
Dr. Wakas Ahmad is board-certified in both Internal Medicine and Emergency Medicine. He received his degree from Midwestern University and his residency was also completed at Midwestern University in combined Emergency Medicine and Internal Medicine. Dr. Ahmad sees patients of all ages at Duly Health and Care Immediate Care Centers in Joliet, Illinois, Lockport, and Oak Lawn.
Internal Medicine:

Dr. Andrew Kalin is board-certified in Pain Medicine and Anesthesiology. He sees adolescents and adults at Duly Health and Care locations in Lockport and Oak Lawn. Dr. Kalin earned his degree at the University of Toledo College of Medicine, completed a residency in Anesthesiology at University Hospitals Case Western Reserve and a fellowship at University Hospitals Case Western Reserve in Anesthesiology.
Palliative Care:
Dr. Melinda Merola practices Palliative Medicine and sees patients of all ages at 430 Warrenville Road, Suite 300, in Lisle. Dr. Merola earned her degree from the Midwestern University Chicago College of Osteopathic Medicine, she completed her residency in Internal Medicine at Advocate Lutheran General Hospital and completed fellowships in Hospice and Palliative Medicine at Midwestern University OPTI at Resurrection Medical Center and Rainbow Hospice Palliative Medicine. Dr. Masse earned his degree at the Loyola University Stritch School of Medicine and completed his residency at University of Chicago Hospitals.
