Charitable Fund

Duly Health and Care’s Char­i­ta­ble Fund invests in solu­tions that improve the health of our com­mu­ni­ties. Togeth­er with our non­prof­it part­ners, we col­lab­o­rate to address the social needs and risk fac­tors that impact health beyond our clin­ic doors.

Our Impact

Found­ed in 2010, the Char­i­ta­ble Fund has grant­ed almost $5 mil­lion to sup­port non­prof­it part­ners with a focus on food secu­ri­ty, afford­able hous­ing and sup­port ser­vices, and access to health­care and com­mu­ni­ty well­ness.

With the help of our donors, the Char­i­ta­ble Fund grant­ed a total of $405,500 in 2023 to 64 non­prof­it orga­ni­za­tions locat­ed across our ser­vice areas.

This includes the 48 Com­mu­ni­ty Grants Pro­gram grantees list­ed below as well as 22 non­prof­it orga­ni­za­tions nom­i­nat­ed by physi­cians and team mem­bers through the Your Cause Grant Program.

Gen­er­ous con­tri­bu­tions from physi­cians, team mem­bers, com­mu­ni­ty mem­bers and busi­ness part­ners are the lifeblood of the Char­i­ta­ble Fund. Duly Health and Care cov­ers the admin­is­tra­tive costs of run­ning the Char­i­ta­ble Fund so that dona­tions can go direct­ly to non­prof­its serv­ing our community.

Our His­to­ry and Structure

Found­ed in 2010, the Char­i­ta­ble Fund was estab­lished by Duly Health and Care physi­cians in part­ner­ship with DuPage Foun­da­tion to pro­vide a coor­di­nat­ed approach of obtain­ing con­tri­bu­tions and giving.

The Char­i­ta­ble Fund oper­ates as a Donor-Advised Fund (DAF) of DuPage Foun­da­tion. As DuPage County’s phil­an­thropic leader, DuPage Foun­da­tion is a 501(c)(3) not-for-prof­it. The Duly Health and Care Char­i­ta­ble Fund is proud to part­ner with DuPage Foun­da­tion to facil­i­tate its giv­ing and grantmaking. 

The Char­i­ta­ble Fund is gov­erned by an Advi­so­ry Board com­prised of physi­cians, team mem­bers, and com­mu­ni­ty mem­bers. The Advi­so­ry Board meets reg­u­lar­ly to review appli­ca­tions and pro­vide grant recommendations.

Prospec­tive grantees must receive an invi­ta­tion to apply for fund­ing. To deter­mine eli­gi­bil­i­ty, orga­ni­za­tions may send basic infor­ma­tion about their pro­grams to the Char­i­ta­ble Fund at charitablefund@​duly.​com.
