Charitable Fund
Duly Health and Care’s Charitable Fund invests in solutions that improve the health of our communities. Together with our nonprofit partners, we collaborate to address the social needs and risk factors that impact health beyond our clinic doors.
Our Impact
Founded in 2010, the Charitable Fund has granted almost $5 million to support nonprofit partners with a focus on food security, affordable housing and support services, and access to healthcare and community wellness.
With the help of our donors, the Charitable Fund granted a total of $405,500 in 2023 to 64 nonprofit organizations located across our service areas.
This includes the 48 Community Grants Program grantees listed below as well as 22 nonprofit organizations nominated by physicians and team members through the Your Cause Grant Program.

Generous contributions from physicians, team members, community members and business partners are the lifeblood of the Charitable Fund. Duly Health and Care covers the administrative costs of running the Charitable Fund so that donations can go directly to nonprofits serving our community.

Our History and Structure
Founded in 2010, the Charitable Fund was established by Duly Health and Care physicians in partnership with DuPage Foundation to provide a coordinated approach of obtaining contributions and giving.
The Charitable Fund operates as a Donor-Advised Fund (DAF) of DuPage Foundation. As DuPage County’s philanthropic leader, DuPage Foundation is a 501(c)(3) not-for-profit. The Duly Health and Care Charitable Fund is proud to partner with DuPage Foundation to facilitate its giving and grantmaking.
The Charitable Fund is governed by an Advisory Board comprised of physicians, team members, and community members. The Advisory Board meets regularly to review applications and provide grant recommendations.

Prospective grantees must receive an invitation to apply for funding. To determine eligibility, organizations may send basic information about their programs to the Charitable Fund at charitablefund@duly.com.
