DOWNERS GROVE, Ill. — Duly Health and Care ACO (“Duly ACO”) has been selected by the Centers for Medicare and Medicaid Innovation (CMMI) as one of 132 participants in the ACO Reach (Realizing Equity, Access, and Community Health) model for 2023. The performance year for this program began on January 1, 2023, and it will run through 2026. Duly ACO will take responsibility for the quality and total cost of care for a defined population of eligible senior patients.
Duly has participated in an Accountable Care Organization program since 2014. Most recently, Duly ACO generated over $24 million in savings in the 2021 performance year in the Medicare Shared Savings Program (MSSP) model. As an organization focused on providing high quality, accessible care for all patients, the ACO Reach model is a natural progression of Duly Health and Care’s value delivered to patients through expertise in Value-Based Care and an integrated outpatient care delivery model. The ACO model exemplifies the way in which Duly’s physicians and care providers deliver exceptional care in each interaction with patients.

At Duly, patients are able to engage in their health and care with the support of our providers and care teams through digital tools intended to make care navigation easier. At our ambulatory surgery centers, patients can undergo outpatient surgeries and receive other services in a high-quality and cost-effective care setting.
