Duly Health and Care ACO Achieves Excellence in ACO REACH Performance

DOWN­ERS GROVE, Ill — Duly Health and Care’s Account­able Care Orga­ni­za­tion (ACO) has been rec­og­nized as a top per­former in the 2023 ACO REACH (Real­iz­ing Equi­ty, Access, and Com­mu­ni­ty Health) pro­gram, excelling in both qual­i­ty of care and total cost of care out­comes. Rank­ing as the 12^th largest ACO in the Unit­ed States, Duly serves 43,000 ben­e­fi­cia­ries, under­scor­ing the organization’s reach and influ­ence in pro­vid­ing care.

In 2023, Duly’s ACO con­tin­ued to deliv­er out­stand­ing results, rank­ing in the top third of Stan­dard ACOs for gross per-mem­ber-per-month (PMPM) sav­ings. Fur­ther­more, the orga­ni­za­tion achieved the 4^th high­est gross PMPM sav­ings among all ACOs with over 30,000 patients, demon­strat­ing its abil­i­ty to effec­tive­ly man­age health­care costs while main­tain­ing high stan­dards of patient care.

A major fac­tor con­tribut­ing to Duly’s suc­cess is patient sat­is­fac­tion. In 2023, Duly ACO was ranked in the top third for patient rat­ings of health­care providers, reflect­ing the high lev­el of trust and sat­is­fac­tion patients have with their Duly providers. Addi­tion­al­ly, Duly’s ACO ranked in the top 40% for time­ly hos­pi­tal fol­low-up appoint­ments, ensur­ing patients receive time­ly and appro­pri­ate care after hos­pi­tal dis­charge — cru­cial in reduc­ing read­mis­sions and improv­ing long-term health outcomes.
