CHICAGO, Ill. – Blue Cross and Blue Shield of Illinois (BCBSIL) and Duly Health and Care announced today that they are working together to bring a new senior-focused care delivery experience to Oak Park and surrounding communities at the Duly Health and Care Oak Park Clinic.
The Clinic, at 1121 South Blvd. in Oak Park, Ill., offers services that help seniors understand their total health and achieve their wellness goals, including physical health and mental acuity.
“This collaboration builds on our longstanding commitment to enhance access to quality, cost-effective, equitable care in the communities we serve,” said Stephen Harris, President, Blue Cross and Blue Shield of Illinois. “This project with Duly Health and Care means that those with Medicare Advantage plans from BCBSIL can access care with a focus on member experience, convenience and affordability.”
The Duly Health and Care Oak Park Clinic is open now and available to BCBSIL Medicare plan members. This value-based care arrangement is designed to meet the evolving and diverse needs and lifestyles of members, with the goal of improving outcomes and reducing overall cost of care.

The Duly Oak Park Clinic is unique because of its senior-centric approach that includes:
- 5,000 square feet of warm and welcoming space
- Both a “health” and a “care” room: two purposeful spaces that are designed for meaningful conversations between physician and patient before the clinical exam
- Staffed by board-certified Internal Medicine physicians who specialize in chronic disease management, preventative care, diabetes, high blood pressure, heart failure, COPD, asthma and more
- Lab and imaging services for comprehensive evaluations offered in the same space
